How soon can I be seen?
Most new patients are seen within 1 to 2 weeks based on availability. Can be seen in as little as 48 hours if available.

Initial evaluations are $500 for 90 minute visit; follow-up visits are $250 for 30 minutes.
Do you provide a superbill for Insurance?
Yes - we can provide a superbill after each visit so you can submit it to your insurance for possible partial reimbursement, depending on your plan. Specifics will be based on your insurance.
Do you provide a Good faith estimate on cost?
Yes, as legally required under the No Surprises Act, as an out of network provider that utilizes sel-pay, we do provide a Good Faith Estimate to annual costs associated with care. These are subject to change, however not without sufficient notice given. Please See " Good Faith Estimate" In footer for more details.

By default, no but with your written consent, we can communicate with your other providers to ensure coordinated care.

Will you share my information with anyone without my consent?
No - we only share information with your written permission, except when required by law (e.g., acute safety concerns for your life or the life of others or if there is suspected abuse in home).

Yes - with your consent, a support person can attend part or all of the visit, either in person or via telehealth. IF you want information to be shared with a 3rd party an Request of Information will need to be completed to allow us to share information with them outside of visits.
